After we book your appointment, we will send you an email with new client forms. Please note new company policy to obtain a credit card to hold your appointment. 

 

Time is valuable, both yours and ours!  Please keep in mind that we require 24 hours' notice if a change arises and you need to reschedule or cancel your appointment.  Our dietitians are available specifically for each appointment.  If something unexpected comes up and you do not call or show up for an appointment, please know that we will charge a $40 fee to compensate the dietitian for work time lost.   We cannot bill your insurance if we do not have a visit with you.

We accept Venmo, PayPal, credit card, cash or check for co-payments and co-payments are due at your scheduled appointment time.

Out of Pocket Payment:

If your insurance does not offer nutrition counseling coverage, we offer time-of-service rates, payable in full at your scheduled appointment. 

Initial 60 minute Consultation: $120

45 minute Follow Up: $90

30 minute Follow Up: $60

We have two locations:

 

33 Mulberry St in Springfield MA 

Mulberry St is located off of Maple St. in downtown Springfield.  Parking is free in the back of the building.  The Pyramid Nutrition Services office is located on the first floor. You may use the side entrance or if needed the handicap ramp entrance.

77 Mill St Suite 233, Westfield MA

Enter through Entrance G and follow signs to the elevator.  We are located on the second floor within Millworks co-sharing space. Handicap accessible. Free parking.
